Dental Plans Offered Through UnitedHealthcare (Options Vary by Location)
HIGH PPO PLAN
Available everywhere. You’ll save the most by using a UnitedHealthcare Dental PPO network dentist, which gives you access to discounted rates and lower out-of-pocket costs.
LOW PPO PLAN
Available in most locations, except for Mississippi, Louisiana, and Texas. As with the High PPO, choosing a UnitedHealthcare Dental PPO network dentist helps reduce your costs.
PASSIVE LOW PPO PLAN
Available in Mississippi, Texas, and Louisiana. In this plan, services provided by an out-of-network provider will have the same coverage as services provided by an in-network provider.
DHMO/ENDP PLAN
This plan utilizes the UHC Exclusive Network Dental Plan (ENDP). Team members must reside within 30 miles of a dentist in the ENDP network. There is no out-of-network coverage for this plan.
HOW TO FIND A NETWORK DENTIST
To find a network provider for the PPO plans or the DHMO/ENDP:
- Log in to myuhc.com, “Find a dentist,” choose “Employer and Individual Plans,” enter your ZIP code or city and state, and select “National Options PPO 30 Network” for the PPO plans or “National Exclusive Network Plan” for the DHMO/ENDP plan
