Voluntary Insurance Options Offered Through Aflac

CRITICAL ILLNESS INSURANCE
Provides a lump-sum payment if you are diagnosed with a covered condition such as cancer, heart attack, stroke, or major organ failure. Includes a $125 wellness benefit. Coverage for your spouse/domestic partner is available, and children are automatically covered if you enroll. You must be enrolled in a medical plan to participate.
ACCIDENT INSURANCE
Pays cash benefits if you experience a covered accidental injury. Includes a $100 wellness benefit. You may also cover your spouse/domestic partner and dependent children.
HOSPITAL INDEMNITY INSURANCE
Pays cash benefits to help cover costs associated with a hospital stay due to sickness or injury. Benefits can be used for medical or non-medical expenses. Includes a $125 wellness benefit. You may also cover your spouse/domestic partner and dependent children.
